Indications
This product is a local anesthetic and anti-arrhythmic drugs. It is mainly used for infiltration anesthesia, epidural anesthesia, surface anesthesia (including mucosal anesthesia during thoracoscopy or abdominal surgery) and nerve conduction block. This product can be used for ventricular premature beats and ventricular tachycardia after acute myocardial infarction. It can also be used for digitalis intoxication, cardiac surgery and cardiac catheter-induced ventricular arrhythmias. This product is usually not effective for supraventricular arrhythmia.

Dosage
1. Anesthesia
(1) Commonly Used Adults:
1 Surface anesthesia: 2% to 4% solution does not exceed 100mg at a time. Injectable doses do not exceed 4.5 mg/kg (without epinephrine) or every 7 mg/kg (with a 1:200,000 concentration of epinephrine).
2 fistula block for labor analgesia: with a 1.0% solution, limited to 200mg.
3 Epidural anesthesia: 1.5% to 2.0% solution in the thoracolumbar, 250 to 300 mg.
4 infiltration or intravenous injection of anesthesia: 0.25% ~ 0.5% solution, 50 ~ 300mg.
5 peripheral nerve block: brachial plexus (unilateral) with 1.5% solution, 250 ~ 300mg; dental 2% solution, 20 ~ 100mg; intercostal nerve (each) with 1% solution, 30mg, 300mg; cervical Infiltration with 0.5% ~ 1.0% solution, left and right side 100mg; paravertebral nerve block (each) with 1.0% solution, 30 ~ 50mg, 300mg is limited; pudendal nerve with 0.5% ~ 1.0% solution, left and right sides 100mg.
6 Sympathetic ganglion block: Cervical stellate nerve with 1.0% solution, 50mg; spinal anesthesia with 1.0% solution, 50-100mg.
7 one-time limit, no adrenaline 200mg (4mg/kg), plus adrenaline 300 ~ 350mg (6mg/kg); intravenous regional block, the maximum 4mg/kg; treatment with intravenous injection, the first time The amount of 1 ~ 2mg/kg, the maximum amount of 4mg/kg, adult intravenous infusion is limited to 1mg per minute; repeat dosing, the interval should not be shorter than 45 ~ 60 minutes.
(2) The amount of children used
Depending on the individual, the total dose of one dose must not exceed 4.0 to 4.5 mg/kg, 0.25 to 0.5% of the solution is usually used, and only 1.0% of the solution is used in special circumstances.
2. Anti-arrhythmia
(1) Routine dose: 1 intravenous injection of 1 ~ 1.5mg/kg body weight (usually 50 ~ 100mg) for the first load of intravenous injection 2 ~ 3 minutes, if necessary, repeat every 5 minutes intravenous injection 1 ~ 2 times, but 1 The total amount within the hour must not exceed 300 mg. 2 Intravenous instillation is usually formulated with 5% glucose injection into 1 ~ 4mg/ml drug droplets or administered with an infusion pump. After the use of a load, intravenous infusion can be continued at a rate of 1 to 4 mg per minute, or intravenous infusion at a rate of 0.015 to 0.03 mg/kg body weight per minute. Older people, heart failure, cardiogenic shock, decreased hepatic blood flow, liver or renal dysfunction should reduce the dosage, infusion of 0.5-1 mg per minute. That is, this product can be intravenously infused with a 0.1% solution, not exceeding 100mg per hour.
(2) Extreme: The maximum load in intravenous injection within 1 hour is 4.5 mg/kg body weight (or 300 mg). The maximum amount of maintenance is 4mg per minute.
Adverse reactions
(1) This product can act on the central nervous system, causing lethargy, paresthesia, muscle tremor, convulsive coma, and respiratory depression and other adverse reactions.
(2) Can cause hypotension and bradycardia. High plasma concentrations can cause slow atrial conduction velocity, atrioventricular block, and suppression of myocardial contractility and cardiac output.
Contraindication 
(1) Prohibit allergies to local anesthetics;
(2) Vescal bans in patients with A-Swiss syndrome (acute cardiogenic cerebral ischemic syndrome), pre-excitation syndrome, and severe cardiac block (including sinoatrial, atrioventricular, and intraventricular conduction block).
Precautions
(1) In the case of non-intravenous administration, it is necessary to prevent accidental intrusion of blood vessels, and to pay attention to the diagnosis and treatment of symptoms of local anesthetic poisoning.
(2) during the medication should pay attention to check blood pressure, monitoring of electrocardiogram, and equipped with rescue equipment; electrocardiogram P-R interval or QRS widened, other arrhythmia or arrhythmia aggravated immediately should be stopped.
